It involves working with a dedicated Support Coordinator who helps you connect with various services and supports, ensuring that you get the most out of your plan. With the guidance of a skilled Support Coordinator, participants can make informed decisions, access the best possible supports, and work towards their goals with confidence. Whether you need help managing daily activities, participating in community events, or achieving personal milestones, Support Coordination ensures you have the support and resources needed to succeed. Levels of Support Coordination

An NDIS plan can include three different levels of support coordination:
  • Support Connection: Helps you connect with informal, community, and funded supports to maximise your NDIS plan and achieve your goals. This level builds the participant’s capacity to manage supports independently.
  • Support Coordination: Assists you in understanding and using your NDIS plan effectively. A Support Coordinator ensures a balanced mix of supports to help the participant’s capacity to maintain relationships, manage tasks, live independently, and participate in their community.
  • Specialist Support Coordination: For participants with complex needs, this level provides specialist assistance to manage and resolve challenges in their support environment. A Specialist Support Coordinator ensures consistent and effective service delivery tailored to the participant’s specific goals.

What You Need to Know

  • Choosing the Right Support Coordinator: Ensure that your Support Coordinator has the skills, experience, and understanding of your specific needs. It’s important to find someone who is a good fit and can effectively help you navigate your NDIS plan.
  • Communication and Collaboration: Maintain clear and consistent communication with your Support Coordinator. Regularly discuss your goals, progress, and any changes in your needs to ensure that your supports are aligned with your objectives.
  • Inclusion of Support Coordination in Your Plan: Your funding for support coordination is based on what is deemed ‘reasonable and necessary’ and will be included in the Capacity Building budget. Your plan will specify the level of support coordination you can purchase, or allow you to choose the level if not specified.
